ICD-10, International Classification of Diseases, 10th Revision. †Reference population was children <1 year of age. ‡Reference population was female patients 15–50 y of age and pregnant. Pregnancy was defined based on ICD-10 diagnostic codes indicating birth, stillbirth, abortion, or miscarriage.
